Health Hub of Gulf and Franklin

The Florida Department of Health in Franklin County partners with a local collaborative group, the Health Hub of Gulf and Franklin, to develop and implement a variety of community health projects. The Health Hub collects and analyzes data to educate and mobilize communities, set priorities, gather resources, and plan actions that improve public health. The primary document guiding this work is the Community Health Assessment (CHA).

The Health Hub of Gulf and Franklin’s vision is to work together to empower Gulf and Franklin Counties to improve quality of life for generations.

The Florida Department of Health in Franklin County continually evaluates the county’s health by measuring multiple data points and compiling results into charts and reports. These data are available on FLHealthCHARTS.gov, a resource provided by the Florida Department of Health.
