The Florida Agency for Health Care Administration (ACHA) requires medical facilities to have a CEMP updated and reviewed annually.
The Florida Health Department is responsible for reviewing CEMPs from the following agencies:
- Home Health Aid
- Home Medical Equipment Providers
- Hospices
- Nurse Registries
ACHA’s Emergency Services page provides additional information regarding this requirement as well as templates to aid in the creation of your local CEMP. Once the CEMP has been created, you may send it electronically to our Preparedness Office. We are no longer accepting paper submissions. If the file is too large, please send it to our offices via a data file CD.
Please Note:Florida has a very broad public records law. Most written communications to or from state officials regarding state business are public records available to the public and media upon request. Your e-mail communications may therefore be subject to public disclosure. If you do not want your e-mail address released, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.
Please contact the Public Health Preparedness Department at (850) 227-1276 for further guidence on where to send your CEMP.
