Health Educators work with community partners to implement Heart Health Plus activities, including blood pressure self-monitoring (BPSM) programs, community health screenings, and process referrals to diabetes, tobacco cessation, and other health and social service programs.
While referrals are provided throughout Franklin and Gulf County, the Heart Health Plus initiative is only in Gulf County, where we collaborate closely with Ascension My Gulf Care, and several other programs.
